I don't have a clue, but I have to comment that that is one heck of a difference in carb height!
I think buick..
What do you know about this intake manifold?

i actually don't think the carbs are at different heights. the carb bases are just sloped forward. why you would want the carbs mounted off level, i have no idea.

sure does look like it's for a Nailhead. the bottom of this page will tell you how to measure the width of the manifold to see which type of engine it will fit:

I was wondering if it was for some interesting application for the motor which mounted it at a severe angle

This manifold is for marine application. That is like a nailhead mounted in a ski boat. These engines usually mounted at rather severe angles because of the nose high attitude these boats ran at.
this is not a nailhead manifold, possibly an early cadillac, or studebaker
You're probably right, Nailhead. Chris Craft "Cobras" used a lot early Cadillac engines, and they were available with dual quads and supercharged, with dual quads. The angle of the carb bases still says marine applications.
Not Nailhead: the Nailhead had two intake ports side by side in middle of the manifold and a single runner on either end.
